Summer Fun 2020 Forms

Please complete the following forms for each child participating  in the Summer Fun Program.  Participants will not be permitted to attend without these forms.  Please also send a copy of your child’s immunization records.  All paper forms must be submitted to Candi Alvarez at calvarez@jfedsnj.org no later than Wednesday, July 22.
